Drug Information on Lopid (Gemfibrozil)

LOPID is a medication that helps regulate lipids in the body. It comes in tablet form to be taken orally. Each tablet contains 600 mg of gemfibrozil. Other ingredients in the tablet include calcium stearate, candelilla wax, microcrystalline cellulose, hydroxypropyl cellulose, hypromellose, methylparaben, Opaspray white, polyethylene glycol, polysorbate 80, propylparaben, colloidal silicon dioxide, and pregelatinized starch. Gemfibrozil is the chemical name for the active ingredient, which has a molecular weight of 250.35 and a solubility of 0.0019% in water and acid. It is stable at normal conditions and has a melting point of 58° –61°C.

Usage for Lopid (Gemfibrozil)

LOPID (gemfibrozil tablets, USP) is used as an additional treatment to diet for two purposes. Firstly, it is used to treat adult patients with very high levels of triglycerides in the blood (Types IV and V hyperlipidemia) who have a risk of pancreatitis and have not responded well to dietary efforts to control their levels. These patients typically have triglyceride levels over 2000 mg/dL and have elevated levels of VLDL-cholesterol and fasting chylomicrons. Patients with triglyceride levels below 1000 mg/dL are unlikely to have a risk of pancreatitis. LOPID therapy may be considered for patients with triglyceride levels between 1000 and 2000 mg/dL who have a history of pancreatitis or recurrent abdominal pain that is typical of pancreatitis. However, the effects of LOPID therapy on the risk of pancreatitis in Type IV patients with triglycerides under 1000 mg/dL have not been well-studied. Secondly, LOPID is used to reduce the risk of developing coronary Heart Disease in Type IIb patients who have not responded well to weight loss, dietary therapy, exercise, and other medications. These patients have low levels of HDL-cholesterol along with elevated levels of LDL-cholesterol and triglycerides. The National Cholesterol Education Program defines consistently low levels of serum HDL-cholesterol below 35 mg/dL as an independent risk factor for coronary heart disease. Patients with significantly elevated triglycerides should be closely monitored when receiving gemfibrozil treatment, as it may increase LDL-cholesterol in some patients. It is important to note that the potential risks associated with gemfibrozil, such as malignancy, gallbladder disease, abdominal pain leading to appendectomy and other abdominal surgeries, increased non-coronary mortality, and a 44% relative increase in age-adjusted all-cause mortality seen with a similar drug called clofibrate, may outweigh the potential benefits in treating patients with elevated LDL-cholesterol only. LOPID is also not recommended for the treatment of patients with low HDL-cholesterol as their only lipid abnormality. The initial treatment for dyslipidemia should be dietary therapy specific to the type of lipoprotein abnormality. Factors such as excess body weight, excessive alcohol intake, and diseases like Hypothyroidism or diabetes mellitus should be addressed before considering drug therapy. Physical exercise and adequate treatment of contributory diseases can also be beneficial. Estrogen therapy may cause a significant increase in plasma triglycerides, particularly in individuals with familial hypertriglyceridemia. Discontinuing estrogen therapy may eliminate the need for specific drug therapy for hypertriglyceridemia. Drug therapy should only be considered when non-drug methods have not been successful, and patients should be informed that drugs do not replace the importance of adhering to a proper diet.
