Isotretinoin USP is a retinoid available as Zenatane in soft gelatin capsules. The capsules come in 10 mg, 20 mg, 30 mg, and 40 mg doses for oral administration. Each capsule contains various ingredients including butylatedhydroxyanisole, edetate disodium, hydrogenated vegetable oil, medium chain triglyceride, refined soybean oil, and white wax. Gelatin capsules contain different pigments depending on the dose. Isotretinoin is chemically known as 13-cis-retinoic acid and is similar to retinoic acid and vitamin A. It is a crystalline powder with a molecular weight of 300.44. The product has passed the USP Dissolution Test 5.
Zenatane is indicated for the treatment of severe recalcitrant nodular Acne. Nodules are inflammatory lesions with a diameter of 5 mm or greater. The nodules may become suppurative or hemorrhagic. "Severe" means having many nodules, as opposed to just a few. Zenatane should only be used for patients with severe nodular acne who have not responded to other treatments, including systemic antibiotics. Zenatane should not be used by pregnant patients because it can cause life-threatening birth defects. A course of therapy for 15 to 20 weeks has been shown to result in complete and prolonged remission of the disease in many patients. If a second course of therapy is needed, it should not be started until at least 8 weeks after completing the first course. It is important for patients who have not completed skeletal growth to be cautious when taking Zenatane because it can affect bone mineral density, cause hyperostosis, and premature epiphyseal closure.
